Thank-you Helen for your 'Like' of this idea. We're developing this nicely and have found a third party provider who is able to 'pull' information from both of our provider's clinical systems and provide it as a 'single-version-of-the-truth'. Even better, this can happen via the web, so no separate bolt on is required. There is really clear clinical governance and the solution also helps with clinical workflow.
We've been developing a DPIA as part of this, and the company already operates in our geography having previously been approved all the way through the DTAC journey.
We see this as a real enabler to help our organisations share information across the whole pathway in a seamless way; and critically will support all our work on clinical role developments/extension of community clinical scope of practice; overseen by the consultant team.
We held a workshop last week on this and everyone very excited!
