What is the challenge your project is going to address and how does it connect to the theme of 'How can improvement be used to reduce delays accessing health and care services'?

People with frailty are often accessing health and social care services at a time of crisis.  This  results in poorer outcomes and additional pressures across health and social care services. This project will focus on one locality and apply an approach that learns from what matters to people using services and those who support them.  There will be a strong focus on addressing inequalities in access to services and supports.
