  • Adapting the hospital passport for patients with dementia

    Many of us will be familiar with the use of hospital passports for those with learning disabilities. At a care home in Scotland, a student nurse has come up with a way of adapting hospital passports more widely to encourage a holistic, person-centred approach for those living with dementia. Carol Dale tells us about this development and the impact it is having on improving system flow across care settings.
