I am working in Bury in Greater Manchester in a Local Care Organisation so integrating health and care is very much what I am involved in. Current work includes further development of neighbourhoods having established MDTs and risk stratification, implementing a single model of Intermediate Care and Rapid Response, a Palliative Care Whole System Flow programme, and developing single line management arrangements across services
