What is the challenge your project is going to address and how does it connect to the theme of 'How can we improve across system boundaries?​

When arranging acute admissions to hospital (Medical, Surgical and Paediatric), primary care physicians (usually GPs, but also allied HCPs) spend a lot of time phoning switchboard and then waiting for the admitting team to answer. The admitting teams are generally busy and sometimes are not able to respond to bleeps, and answering calls involves them stopping the work they are doing and then phoning switchboard, by which time the primary care physician may have given up and just sent the patient to A&E. The admission/assessment request is virtually never declined.
