What does your project aim to achieve?

Our project would work with discharge hubs through our Trusted Assessor Service to provide people with dementia and their families the wrap around support they need to restore and re-establish previous caring relationships provide dementia care modelling and behaviours and support the whole family to live happily and successfully with dementia in their lives.
