You say that ‘No NHS Trust has a quality management system other than in specific, regulated, areas‘ – meaning that full-blown (ISO-compliant?) type management systems with audit trails just don’t exist outside those specific narrow areas, right?

I did a bit of work in this area some time ago. I’ll find it and will share. No response last time I shared it here!
Found it. It’s included in the ‘Documents’ to this SIG from the last time I posted about it. It started life as a QI Maturity Matrix (unfinished) from a Q Exchange project , I think (2018), to which I had an input. I’ve modified it a bit and called it a QMatrix.
