What is the challenge your project is going to address and how does it connect to the theme of 'How can we improve across system boundaries?​

It is worth starting this by saying we are a secure hospital (as this has a huge impact and limits us from utilising other feedback systems that might already exist).

This project will requires consideration of the current means of capturing patient feedback and quality improvement ideas for hearing the voice of those who are unlikely to engage in current opportunities (e.g. complaints, forum, community meetings).

We want to be able to improve our structures and capabilities for learning and improvement by listening to all patients.

Through the new Patient Safety Incident Response Framework we are already considering how we hear from patients, and we can use the PSIRF ethos and culture shift to promote this project as the foundations are similar (listening and involving patients more in decisions about their care).

What does your project aim to achieve?

Patients

  • Allow more equitable opportunities for feedback
  • Improve quality of care
  • improve engagement in recovery
  • Allow to influence QI projects

Staff

  • To hear from all patients
  • To review work streams and processes to ensure recovery is at the heart
  • To monitor recovery and risk reduction better
  • Contribute to assessment of engagement and recovery

How will the project be delivered?

We will work with the QI team to ensure a QI approach is utilised.

The ethos of QI is important in allowing us to measure impact and amend the design as we develop it based on feedback.
