    What to expect for the session:

    • Sam Machen, Associate Director of Patient Safety at University Hospitals Sussex and Honorary Research Associate at UCL in Patient Safety will be discussing the concept of proportionality in PSIRF
      The session will explore how to know when to use a certain response tool, and whether we can use different tools as a ’stacked’ approach. For example, can you use an After Action Review (AAR) when an incident has been identified as a Patient Safety Incident Investigation (PSII)?
    • There will be an opportunity to buddy up with different colleagues from across England, to discuss the following:
      • How has your PSIRF plan been embedded in your organisation thus far?
      • What tools are you using for PSIRF?
      • Is there a clarity on when you should be using specific tools?
      • Where is QI fitting in to this?
