I’m about to start documenting an NHS Standard Process for Discharge Summaries. This will be for a particular Trust and therefor for a particular set of WAD. I undertook a similar task back in 2019 for Walsall Healthcare NHS Trust PAU. It would be great if some of the members of this SIG help by looking at the documentation as the design progresses and comment on the ‘user friendliness’ of the documentation and the process we are taking to arrive at a true representation of the Discharge Summaries WAD. Of particular interest to clinicians will be how the NHS Standard Process documentation differentiates between clinical tasks and operational tasks in the same WAD process. Some/all will be surprised I hope! Details of progress will be posted in the Process Visualisation SIG.

This is a subject that we have been discussing a lot in context of secondary / primary care interface re initiation / change of medication. There seems to be a variety of formats which create increase burden , operational failures and risks .
This is a part of the discharge process that really needs consideration in my opinion.
Yes. One issue in primary care is that patients book GP appointments after discharge and the GP has not yet received any information/discharge summary.
