Biography

In 2011, my daughter Jasmine died following failures in her care.

Ever since, I have been passionate about bringing improvement in Paediatric Patient Safety, Meaningful engagement with parents at the point of care to improve outcomes, and meaningful engagement with bereaved parents to maximise learning from serious incidents when things go wrong.

I also engage in activities designed to improve the 'care package' provided to avoidably bereaved parents after a child dies as a result of medical error.

I am a firm supporter of the Just Culture Model. I believe in restorative justice.
