Hi everyone – please take a look at our Q Exchange idea to reduce harm in older people who are prescribed 10 or more medicines with a focus on reducing medicine waste at the same time and if you like our idea please vote for us 🙂

I wonder if there will need to be a very simple test of cognitive ability in this group when initiating the discussion and recording it. Mild cognitive impairment is common and may need slightly different tactics ? Also follow up of this angle after deprescribing would be interesting.
