    Dear Human Factors group, 

    I work in the QI team at Great Ormond St Hospital, and we are starting a project looking at reducing medication errors – this has come out of a number of incidences both internally and externally. We are wanting to take a ‘human factors’ based approach to reducing errors and improving learning from errors. It would be great if anyone else has worked on similar if we could connect.     • Hi Catherine!
      I like your idea of reducing medication errors, this is something that will benefit us all, specially to prescribers like myself.
      I believe that human factors can influence in our prescribing decisions, I have no doubts on it and it would be interesting to explore that into more detail.
      I haven’t worked on any similar project, but I wouldn’t mind collaborating on it in any way.

      We recently used an AAR to review a medication incident, which was very useful at picking out the learning (including the Human Factors).
