I am a former Paediatric Intensive Care Nurse who has specialised in patient safety for over two decades, mainly at an international and national level. I have also been a clinical risk manager and clinical governance lead in the NHS.  I led the Sign up to Safety, a 6 year campaign to improve the safety culture within the NHS.  I have an honorary DSc and a DProf in Patient Safety as well as an MSc in Clinical Risk. I am a visiting professor for Imperial College and Northumbria University and have written three books; Rethinking Patient Safety, Implementing Patient Safety and Patient Safety Now.  I specialise in safety I and safety II, psychological safety and a just culture.
