Recently participated in Rapid Process Improvement Workshop with the Pharmacy team at the Acute Hospital (2 sites). My role was to examine and question the steps for the patient journey to receiving medication on discharge. This week am taking part in a forum to see how the Integrated Care Communities are improving the joined up thinking for the patient journey. Would it be better for Bob (my late husband) and I to be going on our journey today.
