Different falls prevention in place to reduce harm post Covid

During Covid initial Falls service were stopped.. District Nurse and our Falls Services undertaking less face to face but more telephone contact. Project to review how to work differently

What is the positive change that has emerged through new collaborations or partnerships during Covid-19 that your project is going to embed?

Working with our physios, OT, pharmacy staff has let to a more effective and efficient service for the public exploration of how to put a new model in place is the work we wish to carry out and improve services in both the community and acute.  We have worked with local authorities, commissioners and voluntary sectors and we would like to explore this more as a project with the support of the Q programme.

What does your project aim to achieve?

Improved prevention of falls with early identification of at risk members of the public.  Once identified an individualised programme of care, exercises as due to Covid difficulty in providing exercise classes.  A wider group of health professionals supporting an individual whilst at home.

The Falls Service and health professionals will monitor an individuals ability and on going health needs to ensure they feel empowered and cared for by the NHS

Reduce falls in our region for patients at home and in hospital measure quarterly.

Prevent serious incidents reduce SI’s.

How will the project be delivered?

Patients after self review of risk assessment will be Initial referral by GP or person themselves with links to health professionals.

On going exercises and monitoring by Falls Service especially if linked to a chronic or serious illness pathway with on going monitoring of health and well being both physical and mentally.

Home assessments by OT’s make the home safe, foot wear review;, discuss mobility ensure appropriate mobility aids then put in an exercise programme with on going support.

More telephone calls and Facetime than face to face contact but still giving appropriate support.

Questionnaires to patients and carers especially if nursing homes involved as well to identify any improvements that are required.

How is your project going to share learning?

Through Trust Falls Group and then Regional sharing with commissioners and national NHS England Falls Team.

How you can contribute

  • We will build this into our on going strategy as we are coming to end of our first 3 year strategy for this organisation so will include this in the next 3 years to give adequate time for a whole change in service delivery and provide training for staff across our region.
