What is the challenge your project is going to address and how does it connect to your chosen theme?

To develop first contact practitioner services in primary care using medical region specialist expertise to maximise minimal contact/ maximum response time and offer the most appropriate “opinion and management” first time.

What does your project aim to achieve?

To use the experience and expertise gained with setting up spinal, very successful, first contact services with support of Health Foundation to develop services for shoulder patients. The suggestion is that a similar but distinctly different approach may be needed for shoulder patients, using different personnel and skill mix, we would like to test our assumptions.

How will the project be delivered?

MDT skill mix using various personnel mix to deliver patient self-referral should clinics

In a primary care setting utilising specialist services from secondary care. We aim to maximise opportunities to empower the patients and encourage use of social prescribing opportunities. Part of the project will be to develop an appropriate method of stratification enabling direction to appropriate service first time.

What and how is your project going to share learning throughout?

Learning will be shared throughout our Clinical special interest groups at national conferences and in publications.

Members of our team are working on STP sub-groups, HEE, NHSE, CPEN, CCG’s etc so these provide many opportunities to share the learning
