About this group
If you are interested in Frailty as a Long Term condition and like to apply QI methodology to your work then this is the place for you. The purpose of this group is to collaborate, share and learn from each other, as we strive to improve the care of older adults living with frailty and the services we provide. It’s a multidisciplinary group because we know you can’t provide effective older persons care without the whole MDT. The ethos of the group is “perfection is the enemy of progress” and so we plan, do, study, act to make small improvements every day. Our motto is Henry Forde’s, “Whether you think you can or you can’t, you are right”.
