A paediatric integrated care network for South West London, connecting frontline paediatricians, GPs and anybody working with Children/Young people (CYP) and groups of CYP and families to improve population health
Instead of referring CYP (Children, Young People) to hospital we develop MDT joint Triage meetings GP and consultants together to facilitate the review of referrals and allow direct clinical input.
Establishing weekly virtual TRIAGE meetings jointly with GPs and Paediatric consultants, and other professionals (health visitors, pharmacists etc.), facilitating faster paediatric input/access using existing integrated Together clinics' infrastructure.
