• Chronic Kidney Disease – Improving outcomes for all

  Working across primary and secondary care boundaries, using integrated IT systems, local knowledge and analytics, this project will identify more patients with CKD giving opportunities for better care

• Delivering World-Class Kidney Care Locally – for every CKD patient

  Successful local improvement has already been achieved by better triage and management pathways in CKD- the aim of this project will be to disseminate this over geographical and specialty boundaries.
