I'm a Registered General Nurse and Registered Mental Health Nurse with a doctorate in hospital falls prevention. My clinical career encompassed Nurse Specialist, Directorate Manager and Deputy Director of Nursing posts in acute hospitals, mental health and community hospital settings. I have designed and led successful RCTs, pilot National Clinical Audits, and regional Quality Improvement projects, and held a range of responsibilities related to improving the quality of investigation and learning from patient safety incidents and methods of mortality review. For almost 20 years I worked within national organisations with responsibilities for patient safety, recently retiring from my role as Deputy Director in the national team at NHS England. National roles included safe staffing, cross-agency work on the measurement and assurance of patient safety, oversight of clinical review of all nationally reported death and severe harm incidents, the cross-agency NHS National Patient Safety Alert standards and processes, the NHS Just Culture guide, and strategic patient safety research needs. I now also work as a Non-executive Director in Leeds and York Partnership Foundation Trust. I am a Visiting Professor at University of Leeds with recent publications and work in progress including VTE prevention, suicide deaths, and hospital-level healthcare at home. I am a trustee for NCEPOD.
