During Covid initial Falls service were stopped.. District Nurse and our Falls Services undertaking less face to face but more telephone contact. Project to review how to work differently
In Autumn 2017 identified that we need to reduce/prevent falls in our area to improve outcomes for patients. Falls strategy agreed with a large number of stakeholders. Ideas to implement, care home training including safety huddles and care roundings. Medication reviews in the community with focus on falls and identification of all patients considered a falls risk. Improve the falls clinics and standardise across the catchment area. Implement a sensory awareness network for all staff to have a deeper understand of loss of sight, hearing, being a diabetic, suffering from arthritic or mobility issues.
