Dear Group,
At Great Ormond St Hospital we are looking at starting a Quality Link programme – starting with encouraging inpatient ward nurses/ HCAs to join, hopefully rounds of PDSA cycles to grow this out to other staff groups and areas of the hospital. Would love to connect with anyone who has done a similar project.

I like your idea of reducing medication errors, this is something that will benefit us all, specially to prescribers like myself.

We recently used an AAR to review a medication incident, which was very useful at picking out the learning (including the Human Factors).
