    Hi all, does anyone have any good critical thinking/analytical exercises that I could work through with my team? I have a team of patient safety reviewers doing incident reviews with a number of new people, looking to do some development around investigation/review skills, particularly around getting to the depth of an issue to identify the learning.

    • I have an illustration of the possible elements of the sort of Service re-design called for in the NHS Long Term Plan. It would be great the have the illustration looked at and some critical thinking/analytical review feedback provided. The illustration includes the requirement for patient safety and learning as a key part of the re-design and implementation activities. If you are interested I can add the illustration in the ‘documents’ folder for this SIG.

    • Hi Ashley, I have been thinking a lot about this recently, partly because of Safety-II thinking with the emphasis on how it goes right so often and also related to how we take a more systemic view of what happened when an incident occurred. On the first part, it might be about exploring what is happening when it goes right and so what was missing at the time. On the second, I’m thinking particularly about how people are supported to think about how the multiple and unique conditions interacted at the time and generated what happened. Thinking about the things in between the parts is much harder than the parts individually. Causal loop diagrams spring to mind, or just a well facilitated space to support conversations between people to reflect on the what, so what and now what. The people involved are such a key part of this and how they are supported with compassion to explore their own thinking about what was happening and what their own learning was, in a non judgmental way. Not quite an answer I know, but very happy to talk about this if it is helpful.

      • It’s an interesting area, and for practitioners an area I will be interested to see how it develops. There is quite a nuanced skill in looking at what works and areas for improvement at the same time

    • Hi Ashley, I guess the first one to come to mind would be Deming’s Red Bed Game however there are lots to consider, how about organising a chat so I can understand your team and the needs a little more and then I could give some more tailored advice.
