We are excited to introduce a new digital solution at primary-secondary care interface for the first time in UK. This will facilitate remote monitoring of patients by streamlining blood tests and investigations for secondary care.

We have entered this initiative into the Q Exchange as “Digital integration at primary-secondary care interface to streamline blood tests.”

Patients with chronic illness are followed-up via telephone/video consultations and patients had to come for a separate phlebotomy appointment. Majority of them preferred to have blood tests locally. We plan to roll out electronic requesting of blood tests in the community, to facilitate remote blood sampling ahead of telephone/video consultations. GP practices currently cannot print EPR requests made in secondary care as the IT systems are not integrated. We will provide them a CareAware Connect device linked to our EPR to enable this. The results are returned to their clinicians rather than GPs, enabling prompt endorsement and reducing additional workload on GPs.
