This webinar will:
• Explain how practices have used these tools
• How their care of patients with diabetes was improved
• How it is important to involve patients in any changes
• Describe an available toolkit which contains information and guidance on the tools

The session will be chaired by the College QI Clinical Leads, Dr Bill Taylor and Dr Joanna Bircher. Discussion of the above will be followed by an interactive question and answer session.
